What is actually on a liver panel?

Liver function tests are a group of blood measurements rather than one number, and most of them are not measures of function at all. They are enzymes that leak from liver cells when those cells are stressed or damaged.

How do clinicians read the pattern?

By its shape rather than one value. Two broad patterns cover most results.

Hepatocellular: ALT and AST raised more than ALP. This points to the liver cells themselves, and is the pattern in fatty liver, viral hepatitis and drug injury.

Cholestatic: ALP and GGT raised more than ALT and AST. This points to bile flow, raising questions about gallstones or the bile ducts.

GGT is what distinguishes a bone source of ALP from a liver one. If ALP is up and GGT is normal, the ALP is probably coming from bone rather than the liver, which matters if you are being investigated for bone and liver problems at the same time. Why can AST rise with a healthy liver?

Because AST is present in skeletal muscle. A hard workout, a fall, or a muscle injury can raise it, and so can some cholesterol medicines through their effect on muscle. If AST is up and ALT is normal, muscle is a common explanation rather than a liver problem.

This matters more for women in midlife than it used to, because resistance training is now widely and correctly recommended for bone and muscle. An unfamiliar heavy session in the days before a blood draw is a genuine confounder worth mentioning.

Is the "normal" range actually normal?

This is the most useful thing on this page. Older laboratory upper limits for ALT were derived from populations that included many people with undiagnosed fatty liver, which pushed the apparent ceiling higher than it should be. Several professional bodies now use lower thresholds, and the suggested upper limit for women is lower than for men.

The practical effect: an ALT your report marks as normal can still be worth a conversation, particularly if it has been drifting upward across successive tests. A trend across several results tells you more than one value against a range. Keeping your own record is the only way to see it, because a single report shows today's value without its history.

What most often raises ALT?

  • Fatty liver is the most common cause of a mildly raised ALT. Prevalence in women rises after menopause: estrogen influences how the liver handles fat, and the redistribution of body fat toward the abdomen that accompanies the transition adds to it. It is often reversible, and weight loss of around 5 to 10% has measurable effects on liver fat. See menopause weight gain.
  • Alcohol, which classically raises GGT and often AST more than ALT.
  • Medicines and supplements. High-dose paracetamol, some antibiotics, and several herbal products including green tea extract, high-dose turmeric, ashwagandha and red yeast rice have all appeared in liver injury reports.
  • Viral hepatitis, which is why a persistently abnormal panel gets tested for it.
  • Thyroid and coeliac disease, both of which can present with abnormal liver enzymes. See hypothyroidism symptoms.

Bring the actual supplement bottles to the appointment. A list written from memory usually omits the one that matters.

What monitoring might you be asked to do?

Some treatments require liver blood tests on a schedule rather than only when something feels wrong. The non-hormonal hot flash drugs fezolinetant and elinzanetant both carry that requirement, and it is the step most likely to lapse once a prescription is running smoothly. When does a result need attention rather than a repeat?

  • Jaundice: yellowing of the skin or the whites of the eyes
  • Dark urine with pale stools
  • Persistent pain in the upper right abdomen
  • Unusual tiredness with nausea and loss of appetite
  • Any enzyme raised several times above the upper limit rather than mildly

Questions worth asking: "Is this a single abnormal result or a trend across my previous tests?", "Could any of my medicines or supplements be causing it?" and "Do I need an ultrasound, or do we repeat the blood test first?"

What does a normal panel not rule out?

More than most people assume. Liver enzymes can be entirely normal in significant liver disease, including advanced fibrosis, because the enzymes measure cell injury rather than the amount of scarring already present.

This is why a normal ALT is not a clean bill of health in someone with metabolic risk factors. Where fatty liver is suspected, non-invasive scoring using routine bloods and imaging such as elastography assess fibrosis directly rather than inferring it from enzymes. If your risk factors are there and your enzymes are normal, that is a reasonable question to raise rather than a conversation-ender.

How often should an abnormal result be repeated?

A mildly raised ALT in someone well is usually repeated rather than investigated immediately, because transient rises are common. Repeating after several weeks, with alcohol and recent supplements accounted for, separates a blip from a pattern.

What changes that approach: enzymes several times above the upper limit, a raised bilirubin, symptoms, or an abnormal albumin or INR, since those last two are the actual function measures on the panel rather than leakage markers.

What should you bring to the appointment?

  • Your previous results, so a trend can be seen rather than a single point.
  • Every supplement bottle, not a list from memory. Green tea extract, high-dose turmeric, ashwagandha and red yeast rice all appear in liver injury reports.
  • An honest alcohol figure. GGT is sensitive to it and the conversation goes better when the number is real.
  • Recent exercise, if you have started resistance training or done anything unusually heavy in the days before the draw.

Why do women get investigated differently?

Two reasons worth knowing. Alcohol thresholds used in liver assessment are lower for women than men, because the same intake produces higher blood alcohol and more liver exposure at a given body weight. A weekly amount that would not raise questions in a man may be flagged in a woman, and that is a physiological difference rather than a judgement.

Second, the suggested upper limit for ALT is lower for women. Combined, these mean a result read as unremarkable using older or male-derived thresholds can deserve more attention in a woman.

What happens after an abnormal result?

The usual sequence is repeat bloods with a fuller panel, an ultrasound of the liver if the abnormality persists, and testing for viral hepatitis and autoimmune causes. Where fatty liver is likely, a fibrosis score calculated from routine bloods often comes before any imaging of stiffness.

What it is not, in most cases, is a biopsy. That has become far less common as non-invasive assessment improved, so a persistently mildly raised ALT does not put you on a path toward one by default.
