What does CRP measure, and which test did you have?

C-reactive protein is made by the liver in response to inflammation anywhere in the body. It is not specific: it says something is inflamed, not what.

Two different tests share the name, and confusing them produces an answer to a question you did not ask. Standard CRP detects and follows infection or inflammatory illness, where values run high. High-sensitivity CRP (hs-CRP) measures the low end of the range and is used to refine cardiovascular risk in people who are otherwise well. Asking for "a CRP" without specifying can get you the wrong one.

A single value above 10 is generally not read as a cardiovascular number at all. It suggests something acute, and the test is repeated after recovery.

Which interference is most likely to affect you?

Oral estrogen raises CRP. Estrogen taken by mouth passes through the liver first and increases CRP production directly. Transdermal estrogen, which skips that pass, does not produce the same rise.

This matters because a raised CRP on oral hormone therapy is not evidence of vascular inflammation in the way the risk bands assume. Interpreting the number without knowing the route can turn a pharmacological effect into a false alarm, and can lead to a treatment decision made on a misread. What else moves it?

  • Any recent infection, including a cold, for one to two weeks afterwards
  • Recent injury, surgery or dental work
  • Autoimmune conditions such as rheumatoid arthritis
  • Obesity, which raises baseline levels through fat tissue itself
  • Smoking, poor sleep and periodontal disease
  • Intense unaccustomed exercise in the preceding days

Because so many ordinary things move it, hs-CRP is most useful measured twice, at least two weeks apart, when you are well, with the lower of the two values used. A single reading taken opportunistically during a routine draw is weak evidence either way.

Why does it rise at menopause?

Low-grade inflammation increases across the menopause transition, alongside the shifts in lipids and in where body fat is stored. A modest rise in your forties or fifties is a common finding rather than a separate diagnosis.

What actually lowers it?

The things that lower hs-CRP are the things that lower cardiovascular risk generally: stopping smoking, regular physical activity, weight loss where there is excess weight, treating sleep apnoea, and dental treatment where there is gum disease. Statins lower it too, though they are prescribed for their effect on lipids and events rather than on CRP itself. See statins for women.

There is no supplement with convincing evidence for lowering hs-CRP in a way that changes outcomes, despite a large market built on the claim. Anti-inflammatory marketing is not the same as a measured reduction in events.

When is it worth measuring, and when is it not?

hs-CRP is an additional marker rather than a decision-maker. It is most useful for a person whose cardiovascular risk sits near a treatment threshold, where an extra piece of information could reasonably tip the discussion.

It is not a screening test for the general population. If your risk is already clearly high or clearly low, hs-CRP will not change what happens next, and a low result does not offset high cholesterol, high blood pressure or smoking. Questions worth asking: "Was I well when this was drawn, and should we repeat it?", "Does the form of my hormone therapy affect this result?" and "Does this change what we do, or is it background information?"

Where did the cardiovascular use of CRP come from?

The bands did not appear from nowhere. Large prospective cohorts found that hs-CRP predicted cardiovascular events independently of cholesterol, and the JUPITER trial then randomised people with normal LDL but raised hs-CRP to a statin or placebo and found fewer events on treatment.

That trial is the reason hs-CRP entered risk discussions at all. It is also the reason for the caution that follows: JUPITER tested a statin, not a CRP-lowering strategy, so the finding supports using hs-CRP to identify who might benefit from treatment rather than treating the number itself.

What does a normal result not rule out?

Plenty. A low hs-CRP in someone with high LDL, high blood pressure or a smoking history does not offset those. Inflammation is one contributor among several, and it is the least established of them.

The reverse is also true: a raised hs-CRP with otherwise low risk does not by itself justify treatment. It is a modifier of a risk estimate rather than a diagnosis, which is why it belongs in the middle ground and nowhere else.

How does this connect to the rest of a midlife work-up?

hs-CRP is rarely useful alone. It becomes informative alongside a lipid panel, a blood pressure record and a fair account of activity, sleep and smoking.

Two specific overlaps worth knowing. Obesity raises baseline hs-CRP through fat tissue itself, so weight change moves the number without anything happening to arteries. And untreated sleep apnoea, more common in women after menopause than is generally recognised, raises it as well; see menopause and sleep.

Periodontal disease belongs in the same list and is the one most often left out. A dental appointment is a reasonable response to an unexplained raised hs-CRP.

Is CRP the same as an ESR?

No, though they are often ordered together and both reflect inflammation. CRP rises and falls quickly, within hours to days, so it tracks what is happening now. The erythrocyte sedimentation rate changes slowly over weeks and is influenced by age, anaemia and pregnancy independently of inflammation.

In practice, a raised ESR with a normal CRP often points to something chronic or to one of those confounders rather than acute inflammation. If you have both on a report, they are answering slightly different questions.

What if your hs-CRP is persistently raised and nobody can explain it?

The order of investigation usually runs: confirm it on a repeat when well, check for the obvious contributors including obesity, smoking, gum disease and sleep apnoea, then consider whether an inflammatory or autoimmune condition is being missed.

Persistent elevation without an explanation is uncommon and is a reason for a broader look rather than for treating the number. What it is not is a reason for an anti-inflammatory supplement regimen, since none has evidence for changing outcomes.

Should you pay for it if it is not offered?

Direct-to-consumer panels increasingly include hs-CRP, often bundled with a long list of markers, and the marketing frames it as an early warning system. It is worth being clear about what you would be buying.

If your cardiovascular risk is already clearly high or clearly low, the result will not change what happens next. If you have not yet had your blood pressure measured properly, your lipids checked, or an honest conversation about smoking and activity, hs-CRP is a long way down the list of things worth paying for. Those come first because they carry more weight and are more readily acted on.

There is also the practical problem of a single unexplained result arriving without a clinician attached to it. Given how many ordinary things move hs-CRP, an isolated raised value from a home panel most often generates anxiety and a repeat test rather than a decision.

Where it earns its place is the middle ground: a woman whose calculated risk sits near a treatment threshold, where one more piece of information could reasonably tip the discussion. That is a conversation to have with whoever would act on the result, before ordering it rather than after.
